E-log book for general medicine

  This is an online E-log entry blog to discuss and understand the clinical data analysis of a patient, to develop competency in comprehending clinical problems, and providing evidence- based inputs in order to come up with a diagnosis and effective treatment plan to the best of my ability. A 30 year old female who is a resident of Nagarjunasagar and a homemaker was brought to the casualty with chief complaints of involuntary jerky movements in all 4 limbs. History of presenting illness  17th March,2022 : 12 days ago she developed fever which was insidious in onset, high in grade, gradually progressive and subsided on taking medication. It was accompanied with an occasional dry cough. 18th March,2022 : She was taken to a local clinic the next day where the NS1 antigen test was ordered. A 70 year old male who is a daily wage labourer by occupation was brought to the casualty ward with the chief complaints of altered sensorium since 3 days and fever, shortness of breath and a productive cough since 23 days Timeline :  23 days ago : The patient was apparently asymptomatic 23 days ago when he went on a sudden alcohol binge for a couple of days following which he developed cough assosciated with sputum (scanty, non foul smelling, non sanguinous) and shortness of breath that could initially be classified under Class 2 NYHA(SOB with ordinary activity, slight limitation of physical activity) but gradually progressed to class 4(SOB at rest, severe limitation of physical activity). ...

 This is an online E-log entry blog to discuss and understand the clinical data analysis of a patient, to develop competency in comprehending clinical problems, and providing evidence- based inputs in order to come up with a diagnosis and effective treatment plan to the best of my ability. 53yrs old female who is homemaker and resident of Cherlapalli complains of giddiness since 3 days and abdominal pain since 3days  Case timeline: JUNE 2010:  Patient experienced giddiness following which she lost consciousness and attenders noticed a deviation of her mouth to right. No seizures were noted. She was taken to a local hospital and was admitted in ICU where high blood sugar was detected and the patient was duly informed regarding her condition.
